CRC Case Managers are Registered
Nurses who have attained a national certification in case management. Our case managers specialize in Geriatrics, Traumatic
Brain Injury, Catastrophic Injury, Heart Disease, Diabetes, Asthma, Stroke, Orthopedic Trauma, Pediatrics, Developmental Delays,
Rare and Chronic Conditions, Oncology, Dialysis, Transplants, Chronic Pain, to name a few of our medical specialties.
CRC Case Managers embrace the philosophy that clients must take control of their injury, illness or disability.
CRC Case Managers incorporate healthy lifestyle interventions to maintain restored health. What will my Case Manager do? Collect and review information about medical history, current medical problem, insurance and other funding
sources, current physicians, other health care providers, family, significant others, education, type of work, lifestyle,
culture, etc.
CRC Case Mangers review this information to create a care plan
that fast tracks the client to optimum health and independence. The care plan documents long and short-term goals, interventions,
and approximate periods to achieve these goals. The Care plan tracks achievements and setbacks. The care plan documents new
interventions and outcomes. The care plan includes healthy lifestyle recommendations and educational material. THe Care
Plan may recommend referrals to appropriate specialist. The care plan is road map for the client, significant others,
the health care team, insurance carriers and employers.
- Facilitate and Coordinate
The case manager interacts and communicates with health care providers by onsite meetings,
telephone calls, and correspondence. In addition to the health care providers, the case manager interacts and communicates
with the client, families, significant others, attorneys, specialty program providers, insurance company, employers and
others participating in the client's recovery. The interaction and communication initiates when the case opens and
ends after the client is transitioned to independence or to other supports and the case is closed.
The CRC Case Manager is an advocate for the client and his family.
Each client is treated with respect and dignity. Each client is the center of care plan. Each client is empowered to gain control of his illness, injury or disability. The care plan is created with input from the client and other members
of the team. The Case Manager monitors the insurance
and other funding sources to be sure it used in an appropriate cost effective manner. The CRC Case Manager researches other funding sources, community resources, compassionate funding,
programs, and entitlements that benefit the client. The
CRC Case Manager strives for open communication with everyone on the team. If communication is comprised the CRC Case Manager institutes
interventions to open and restore the communication. Prior
to closing the case, The CRC Case Manager ensures there is a solid plan to transition the client from case management to independent
care, or to the care of others. Let
